Chain of Custody Form

Many clients have a chain of custody (COC) form which is submitted with the sample. A form is attached for clients not using such a system. Note; it is essential that the COC form travel with the sample. It may also be faxed to 02 6247 0982. A purchase order number is essential prior to the analysis being done.
